Insurance
At EYEMKR, we want to make accessing custom prosthetic eyes and prosthetic eye services as smooth as possible. If you choose to use insurance for your prosthetic eye services, here’s what you can expect:
✔ We will work with most Insurance Plans
EYEMKR is willing to work with select insurance providers. If you’re unsure about your coverage, our team will gladly verify your benefits and obtain preauthorization before your first appointment.
✔ Courtesy Billing
We offer courtesy billing to your insurance company on your behalf. This means we’ll prepare and submit the claim directly, so you don’t have to navigate the paperwork alone.
✔ What You May Be Responsible For:
- Deductibles & Copays: Any amount not covered by your insurance plan (such as copays, deductibles, or non-covered services) will be your responsibility.
- Out-of-Network Coverage: If EYEMKR is not in-network with your plan, you may be responsible for a larger portion of the bill.
✔ Documentation Needed
In most cases, your insurance will require a Letter of Medical Necessity (LMN) from your referring provider. We will help guide your doctor’s office with a template to make this step simple.
✔ Process Timeline
Insurance processing can take several weeks to several months depending on the plan and requirements. If you would like to start sooner, you always have the option to self-pay and seek reimbursement from your insurance later. We will provide you with a super bill.
