- Refer your patient to EYEMKR, LLC by providing their contact information and relevant medical history. Email sparkle@eyemkr.com and/or call/text (530)565-6668.
- Our team will reach out to schedule an initial consultation to understand the patient's needs and discuss the prosthetic process.
Step 2: Letter of Medical Necessity (LMN)
- If your patient wants to file reimbursement with their insurance for the prosthetic eye, a Letter of Medical Necessity (LMN) from the referring doctor is required.
- We provide an easy-to-use LMN template to assist you in drafting this important document for your patient who plans on using their insurance for financial assistance.
*WE DO NOT BILL INSURANCE. WE ARE A FEE FOR SERVICE PROVIDER. WE PROVIDE THE PATIENT WITH A SUPERBILL.
Step 3: Assessment and Customization
- During the fitting, we will conduct a thorough assessment and take detailed measurements to ensure a perfect fit.
- Our Ocularist will then create a custom prosthetic eye that matches the patient’s natural appearance and provides optimal comfort.
step 4: Final Fitting and Follow-Up
- Once the custom-made prosthetic eye is ready, we will schedule a final fitting appointment to ensure it meets the patient’s expectations.
- We offer routine maintenance services, including cleaning and polishing, to keep the prosthetic in excellent condition.
